Package Order Form This form allows you to order multiple prints of the same image from any event, at a discounted price. One form per event please. Photo CD Order Form This form allows you to order the image files from any event to print at your leisure. Copyright release forms included with every CD. Adult Model Release Form This form must be filled out if you are of age 18 or over so that I may use any images containing your face for publicity purposes. Minor Model Release Form This form must be filled out if you are, or your child is, under the age of 18 so that I may use any images including your, or your child's, face for publicity purposes.
